Our Community Partners

None of us can do this work alone. The key to our success is our collaborative approach and ability to connect family members to essential community resources. Our partners include:

Westchester County

Family Ties is honored to serve our communities on behalf of the County through our Resource Centers and Every Kid Counts program.

ACHIEVE (NYS System of Care)

An innovative process designed to improve the lives of youth with complex behavioral health needs, and support their overall well-being at home and in their own communities

Crisis Stabilization Project

A collaborative approach to stabilizing youth at risk for behavioral health crises through short-term Mental Health Consultation, Intensive Crisis Respite Support, and Family Peer Advocacy.

Behavioral Health Care Collaborative

Family Ties is an affiliate of Coordinated Behavioral Health Services Behavioral Health Care Collaborative (CCN/CBHS). CBHS is an inclusive and clinically integrated network of behavioral health, physical healthcare, Home and Community Based Service (HCBS) providers, and Community Based Organizations working together in order to improve the quality, cost-effectiveness, and efficiency of health care delivery in the New York Hudson Valley Region.

Performer Providing System (PPS)

Family Ties is a partner in the 2 local PPSs, which are a group of providers who have come together to improve population health by delivering patient-centered, community-based healthcare.

Families Together in New York State

We are the Westchester County chapter of Families Together in New York State, a family-run organization that represents families of children with social, emotional, behavioral and cross-systems challenges. Our goal is to ensure that ALL children and youth have the support they need in order to succeed. FTNYS represents thousands of families from across the state whose children have been involved in multiple service systems including mental health, substance abuse, special education, juvenile justice, and foster care.
