Medicaid & Specialized Services

Families with a child at high-risk, in placement, or returning from placement often need specialized support and assistance. These short-term, intensive programs are designed to provide caregivers with the tools they need to keep their children functioning successfully at home and in the community. Participation is voluntary and by referral only.

ACHIEVE

ACHIEVE (aka New York State Systems of Care) is a four year, state-funded demonstration project convened by the New York State Office of Mental Health (OMH).  This innovative process is designed to improve the lives of youth with complex behavioral health needs, and support their overall well-being at home and in their own communities. The ACHIEVE Project serves 20 high-needs youth and their families on an annual basis.

Crisis Stabilization

An innovative and collaborative approach to stabilizing youth at risk for behavioral health crises through short-term Mental Health Consultation, Intensive Crisis Respite Support, and Family Peer Advocacy.  Crisis Stabilization services are available to youth and their families at home, in school, and in the community.

Every Kid Counts

Sometimes families face a crisis and do not know what to do, or where to turn for help. Before they know it, things can become unmanageable.  As a result, Child Protective Services (CPS) may get involved.  Often, families simply need guidance, support, and access to the right resources to get back on track.  Every Kid Counts is a voluntary, short-term program designed to strengthen family functioning and prevent future CPS reports.

Every Kid Counts is funded through the Westchester County Department of Social Services and serves 60 families per year. 97% of families avoid future involvement with CPS.

Medicaid Services

Family Ties has been designated by New York State to provide Home and Community Based Services (HCBS) for eligible adults and children who are Medicaid recipients.  These include:

Children and Family Treatment and Support Service (CFTSS)/Children’s Home and Community Bases Services (HCBS)

  • Family Peer Support Services
  • Caregiver/Family Support Services
  • Youth Peer Support Services
  • Psychosocial Rehabilitation Services
  • Prevocational Services
  • Community Self-Advocacy and Training
